In real life, people don't use names like this when there's no uncertainty about whom they're addressing, do they?
No, but they do it a lot on TV and in movies, to give the viewers a chance to learn and remember the names, I think. (Heh -- miss_zedem thinks it's an American thing; I think it's a TV thing. Perhaps a combination?) It seems to be very much part of the speech patterns of DS, which I think is why it ends up in fic so much, but yeah, I always try to cut down if I can. You don't need it in fic the same way you do in canon.
Though, otoh, in canon, you can often tell who someone's talking to by eyeline/body language, whereas in fic you have to somehow make that explicit.
